Who we are
This website is run by Active Dental Laboratory Ltd, trading as Active Dental and Active Lab, at 633–635 Birchwood Boulevard, Birchwood, Warrington WA3 7QU.
We are responsible for the personal information you give us through this website. In data protection law, that makes us the "data controller".

What this notice covers
This notice explains what happens to your information when you use this website, send us a booking request, or contact us by phone or email.
If you become a patient, your dental records are covered by our patient privacy notice, which we give you at your first appointment.
What information we collect
When you send a booking request, we collect:
- your name
- your phone number
- your email address
- the treatment you are interested in
- anything you choose to tell us in the "Anything we should know?" box
You might tell us about your health in that box, for example about your teeth or a medical condition. Health information is especially sensitive, so we treat it with extra care. Please only share what you are comfortable with. We will ask for full details at your appointment.
If you phone or email us, we keep the details you give us so we can help you.
How we use your information, and why we are allowed to
We use your information to:
- reply to you and arrange your consultation
- prepare for your appointment
- keep a record of what was agreed
The law allows us to do this because you have asked us to contact you about treatment. This is known as taking steps at your request before providing a service.
If you tell us about your health, the law allows us to use it because it is needed to provide you with health care.
We will never sell your information, and we will not send you marketing unless you have asked us to.
How long we keep it
If you send a booking request but don't become a patient, we delete your details after 12 months.
If you become a patient, your details become part of your dental records, which we must keep for set periods under professional guidance. Our patient privacy notice explains this.
Your rights
You have the right to:
- ask for a copy of the information we hold about you
- ask us to correct anything that is wrong
- ask us to delete your information
- ask us to stop or limit how we use it
- object to how we use it
- ask for your information in a format you can take to another provider
To make a request, email info@activedentalcheshire.co.uk or call 01925 652959. It is free, and we will reply within one month. Some rights have limits; for example, we may need to keep dental records for a set time. If so, we will explain why.
If you are unhappy
Please tell us first and we will do our best to put things right.
You also have the right to complain to the Information Commissioner’s Office (ICO), the UK regulator for data protection. Visit ico.org.uk or call 0303 123 1113.
Changes to this notice
We will update this notice if we change how we use your information, for example if we add new features to the website. The date at the top shows when it last changed.
